Even with good prenatal care coverage, there are still shortcomings in testing and timely treatment, influenced by social, territorial, and access factors, especially in the North region. In this context, spatiotemporal analysis becomes fundamental to identifying patterns and guiding more effective health actions. Objective: To analyze the spatiotemporal distribution of syphilis cases in pregnant women in the 1st Regional Health Center of the State of Pará. Methods: This ecological, descriptive study, with a quantitative approach, analyzed 5,611 confirmed cases of SG (Severe Acute Respiratory Syndrome) residing in the 1st Regional Health Center of Pará, between 2019 and 2024. Data were obtained from the SESPA and subjected to descriptive analyses and statistical tests (Fisher and Kruskal-Wallis), with a significance level of p ≤ 0.05 using the Statistical Package for the Social Sciences (SPSS) v25. Detection rates were calculated using maps, time series, and geoprocessing techniques such as Kernel Density Estimation (KDE). The analysis included Moran's Global Index and the Spatial Scan Statistics (Scan) to identify risk clusters. Results: A continuous increase in cases of SG (Severe Acute Giardia) was observed, especially after 2021. Young pregnant women, residing in urban areas, of mixed race, and with completed secondary education predominated. Most diagnoses occurred in the 3rd trimester of pregnancy, and concomitant treatment of partners was insufficient. Spatial analyses showed an expansion of cases and an increase in rates in all municipalities, with higher concentrations in Belém and Marituba, in addition to the identification of high-risk clusters between 2022 and 2024 in Benevides and Marituba. Conclusion: The potential of spatiotemporal analysis to enhance surveillance of gestational syphilis (GS) and to support health planning is evident. There is a clear need for more targeted actions that are sensitive to local specificities. Thus, addressing this condition depends on improving care and management strategies, with a focus on the effectiveness of interventions and the reduction of inequities.PPGENF - Enfermagem em saúde pública e epidemiologia de doenças na Amazônia
