Objectives: To assess cardiac autonomic modulation, quality of life, and their relationship with inflammation in individuals with Long COVID. Methodology: A cross-sectional observational study conducted between 2020 and 2025, involving 128 adults with a confirmed diagnosis of Long COVID. The sample was selected by convenience, and data were collected through assessment of HRV, administration of the SF-36 and the Post-COVID-19 Functional Scale, and cytokine analysis, in addition to screening for sociodemographic and clinical data. The study was conducted in accordance with the STROBE guidelines. A p-value of <0.05 was adopted as the threshold for statistical significance. Results: Individuals with greater functional limitations had higher concentrations of IL-17A. The cytokines IL-6, IL-17A, IL-4, and IFN-γ were associated with functionality and the physical domains of quality of life, while changes in the RR, SDNN, RMSSD, and SD1/SD2 indices were primarily related to the mental domains of the SF-36. Correlation analysis revealed associations between inflammatory markers, cardiac autonomic modulation, and quality of life. Conclusion: The findings reinforce the multifactorial nature of Long COVID, demonstrating that persistent systemic inflammation and alterations in autonomic modulation act in a complementary manner to impair functionality and quality of life, with predominant associations with different domains.PPGREAB - Avaliação e intervenção terapêutica nos sistemas neuromuscular
