Considering its economic and social implications, this condition is considered a major public health challenge. Although the World Health Organization recommends more effective surveillance practices for CKD, there is a scarcity of research on this topic in the Amazon region, a place of great social and health inequities, which justifies this study. Objective: In this sense, the study aimed to evaluate the clinical aspects of renal function in patients with hypertension and diabetes mellitus treated in primary care in the municipality of Santarém, Pará. Methodology: This is an epidemiological, observational, longitudinal and prospective study, with five (05) Basic Health Units in a municipality in the interior of the Brazilian Amazon as the setting. The target audience was individuals with diabetes mellitus and hypertension enrolled in the HIPERDIA program. The research consisted of a semi-structured interview to collect sociodemographic data and preexisting clinical conditions, guided by the validated instrument "Screening for Occult Renal Disease" (SCORED), in addition to the analysis of biological samples to measure creatinine and estimate the Glomerular Filtration Rate (GFR) of the users. For data analysis, a combination of descriptive and inferential statistical methods was used, employing the RStudio® software, version 4.5.1. The descriptive analysis included mean, standard deviation, absolute and relative frequency, to visualize the distribution of demographic and clinical data. To verify the association between variables, significance tests such as Spearman's correlation test, Fisher's Exact Test, Pearson's Chi-Square Test, and multivariate logistic regression were applied to evaluate independent predictors for renal failure. The Mann-Whitney U test was used to assess the difference in GFR between the groups with and without proteinuria, and the paired t-test was used to assess the mean GFR of the patients. Hypothesis tests with p < 0.05 were considered statistically significant. Results: The study included 263 individuals, of whom 76.43% were female, with a mean age of 63.18 years (SD ± 10.21). Alterations in GFR were identified in 17.87% of the participants. The duration of exposure to the underlying disease did not show a significant correlation with renal dysfunction. A significant difference in baseline GFR was also observed according to sex (p<0.001), with lower means in women. However, men showed a polarized distribution with a relevant frequency in stage 3 (14.52%). Final considerations: This study has limitations, such as its geographical scope restricted to the urban area of the municipality, which may not accurately reflect the reality of kidney health among users of the HIPERDIA Program throughout the municipality, especially in rural and riverside areas. Therefore, it is recommended that future research adopt long-term longitudinal designs, incorporate environmental, occupational, and social variables, and expand the investigation to rural and riverside populations, in order to deepen the understanding of CKD in the Amazon and support more effective prevention and care strategies within the scope of Primary Health Care.PPGENF - Enfermagem em saúde pública e epidemiologia de doenças na Amazônia
